Injury Report
An Injury Report must be completed in its entirety. If anything is on the form is incomplete we cannot send it to Hockey Alberta and it will be returned to you.
Please have a team official complete the form in its entirety including the Doctor/Dentist or Physician's section. Submit to RAIDERS admin at officeadmin@raidershockey.ca for furtherance to Hockey Alberta. Reports must be submitted to Hockey Alberta within 90 days of the accident so please do not wait to complete the form and send it into us.
